Ayabi Home Care LLC is committed to providing quality home care services to our community.  With over twenty plus years of experience in the administration and delivery of home care services, we are committed to providing the best care for our clients in the comfort of their own homes. 

Personal Care Assistance (PCA) Services:

Personal Care Assistance (PCA) services is a program that provides services to elders and people with disabilities to help remain independent in the community. PCA workers provide services in a person's home or in the community. 

Activities of Daily Living (ADL's)

 A PCA may assist the person with the following ADLs:

  • Dressing 
  • Grooming 
  • Bathing
  • Eating
  • Transfers
  • Mobility 
  • Positioning 
  • Toileting

Instrumental activities of daily living (IADLs)

 A PCA may assist an adult recipient with the following:

  • Accompany to medical appointments
  • Accompany to participate in the community
  • Assist with paying bills
  • Communicate by telephone and other media
  • Complete household tasks integral to the PCA services
  • Plan and prepare meals
  • Shop for food, clothing and other essential items

Observation and redirection of behaviors

 A PCA service may include observation of a person who:

  • Has episodes of behaviors
  • Needs redirection to remain safe in his/her environment

To be eligible for PCA services you must:

Live in a home setting and not in a facility setting.

Be able to direct care or have a representative who can direct care on their behalf.

Be enrolled in one of MN health care programs (Medical Assistance, MA Waiver/Alternative Care)

Have an assessment that determines they are eligible.

To be a PCA/DIRECT SUPPORT worker you must:

Be 16 years of age or older.

Pass the standardized certification test.

Complete and pass the background check and application process.

A person employed as a PCA/Support worker must follow through with ongoing requirements

Home Management Services:

Home Management Services may include assistance with:

 

·  Arranging for transportation.

·  Preparing meals.

·  Shopping for food, clothing and household supplies.

·  Performing simple household repairs. 

Coming soon: 245D basic waivered services

245D Basic Waivered Services:

The Department of Human Services (DHS) licensed certain Home and Community Based Services (HCBS) provided to people with disabilities and those over age 65. Ayabi Home Care has applied for 245D Basic Services and will integrate these services once approved.

COMMUNITY FIRST SERVICES & SUPPORT (CFSS)

Community First Services & Supports transition:

Beginning October 1, 2024, DHS will transition people who use Consumer Support Grant and personal care assistance (PCA) services to CFSS.


Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS will replace personal care assistance (PCA) and the Consumer Support Grant (CSG).
