Ayabi Home Care provides Personal Care Assistance (PCA) and Community First Services and Supports (CFSS) to help people remain safe and independent in their homes and communities.

Minnesota is transitioning people from PCA to CFSS. Both programs provide support based on each person’s assessed needs and approved service plan.

Services may include assistance with:

• Dressing, grooming and bathing
• Eating and toileting
• Mobility, positioning and transfers
• Meal planning and preparation
• Laundry and household tasks related to the person’s care
• Shopping and other essential activities
• Medical appointments and community activities
• Health-related tasks
• Observation, supervision and redirection of behaviors

Caregivers may provide hands-on assistance, supervision or reminders based on each person’s individual needs.
